Surface piercing - bar or dermal anchor? - surface bar piercing

I really want a piercing on the neck and tried to get the piercing in a bar when my piercer told me about drilling a point, anchor or the skin.

Could someone please let me know it would be better .. would affect the less, look better, etc.

So, I know that the dermis is anchored permanently, but the removal process (if I) a couple of years really so bad?
